Per RSU 34 Policy - All athletes must have 1 physical on file per the 4 years. This means that you have 1 sports physical done while attending LMS and 1 sports physical done while attending OTHS.

For each Season the following forms must be filled out:

• Form A (Green Form) - Form A - Permission to Participate - Required by all athletes and guardians to fill out - 1 per season required to participate.

• Form B (White Form) - Form B - Athletic Physical Form - 1 Sports Physical per 4-year enrollment at LMS or OTHS

• Form C (Yellow Form) - Form C - Health Update Form - This is filled out if your health has changed since your Sport Physical

• Form D (Pink Form) - Form D - Physical Waiver Form - This form is to be used if a Sports Physical cannot be completed before the start of a sports season. This is a one-time form for all students in sports. You are still required to get a sports physical to continue participating in athletics for RSU 34.
