Form A- Participation to participate form - Required once per year.
Form B- Proof of a physical form- Required once per 3 years while attending LMS.
Form C- Health update form- Required once per year.
Athletic Trainer Release Form- Allows district athletic trainer to administer concussion testing and injury evaluation- Required once per year.
Physical Waiver Form- This form is to be used if a Sports Physical cannot be completed before the start of a sports season. This is a one-time form for all students in sports. You are still required to get a sports physical to continue participating in athletics for RSU 34.
